Do not send paper forms or checks for Company License Application Packages or Company Renewal Forms. Company Renewal Forms can be found in the LSBPSE eLicensure Portal in your application section when it is time to renew.

If you are looking for Company License Application Packages, please contact the Company & Instructor License Section at (225) 272-2310, ext. 223 or via email. Once it has been verified by the section that your company meets the proper requirements to be licensed, the application package will be pushed to your account in the portal.
